Have you thought about just shaving the dog down?
The one tool in the arsenal that was safely at home and I wished I had about a thousand times was a sawzall. Then all I would need would be a masonry blade for it. The only problem you might run into is the fact you can only make the hole an inch and a half wider then you are hitting the studs on either side.
As I was prowling the the hardware stores I did notice they had an insert you can buy rather than going with the plywood I used. It was 28 dollars and went with a more expensive door, but it might solve your width problem.
Then again there is the Euthanasia solution. Or maybe Jingles is still training you to open the patio door?
